1. We will reimburse you for your AATA membership fee (one-time), after you send us 7 patients, that we successfully onboard with our program.
2. Patients must be approved and all documents must be signed by January 31, 2017.
3. We will provide an American Express gift card in the amount paid as a membership fee to the AATA - not to exceed $995.00.
